How Smile Up Dental Clinic Collects, Uses and Discloses Patients’ Personal Information. Our office understands the importance of protecting your personal information. 

To help you understand how we are doing that, we have outlined here how our office uses and discloses your information. The office will collect, use and disclose information about you for the following purposes:

• To identify and ensure continuous high-quality service 
• To provide health care 
• To advise you of treatment options 
• To enable us to contact you 
• To establish and maintain communication with you 
• To offer and provide treatment, care, and services related to general dentistry and oral and maxillofacial surgery.
• To communicate with other treating health-care providers, including dental and medical specialists and primary care providers.
• To maintain communication with you, share health-care information, and book or confirm appointments.
• To follow up efficiently about treatment, care, billing, teaching, and anonymous demonstration purposes.
• To complete and submit dental claims for third-party adjudication and payment.
• To comply with legal and regulatory requirements, including timely delivery of charts and records to the Royal College of Dental Surgeons of Ontario when required under the Regulated Health Professions Act.
• To comply with voluntary agreements or undertakings with the Royal College of Dental Surgeons of Ontario and the Canadian Dental Hygienist Association, including timely delivery or review of patient charts and records for regulatory and monitoring purposes.
• To provide your charts and records to the dentist’s insurance carrier when appropriate.
• To invoice for goods and services, process credit card payments, and collect unpaid accounts.
   

Our office will not disclose your confidential medical history to your insurer under any circumstances. If such a request is made, we will forward the information directly to you for review and specific consent. When unusual requests are received, we will contact you for permission before releasing information and may advise you if we believe the release is inappropriate.
